Propay goals are due to your PLC-F by the end of the day May 24th.   The PLC-F 
will be reviewing them on May 25th and passing them along to Don and the 
district evaluators by May 28th.   I have attached the forms for you to fill out 
as you analyze your data.   Please note below which form you need to use based 
on how you wrote your goal. 
 
If you wrote your goal using NWEA MAP data, there are specific guidelines 
outlined in the attached documents.  The two documents you will use are titled 
“Evidence of student achievement using MAP data 09.10 and NWEA Map data 
guidelines”.  
 
If you did not write your goal using NWEA MAP data, then you will fill out the 
document titled “Evidence of Student Achievement Form 09.10”.
 
Please put items in this order in your PLC-F box by May 24th.
